NDIS & DISABILITY SUPPORT

Supporting Choice, Independence and Participation

RASAK CARE provides disability support services designed around each person’s individual needs, abilities, preferences and goals.

Our approach focuses on supporting people to maintain and develop independence, participate in everyday life, connect with their communities and pursue goals that are meaningful to them.

Support arrangements are developed according to the person’s circumstances, support requirements, applicable funding arrangements and the services RASAK CARE is appropriately able to provide.

Support for More Complex Needs

Some people require additional health-related or specialist support as part of everyday life.

RASAK CARE’s objective is to provide these services safely and appropriately, with suitably qualified or trained professionals where required.

Community Nursing Care

Community-based nursing support delivered according to individual clinical needs and by appropriately qualified nursing professionals.

We recognise the importance of working collaboratively with children, parents, carers and other professionals involved in a child’s support.

High Intensity Supports

Some participants require supports associated with complex health or personal care needs.

These supports require appropriate assessment, planning, worker training and competency.

RASAK CARE will only provide high intensity supports where the required clinical oversight, worker competencies and safeguards are established.

We recognise the importance of accessible information and culturally appropriate communication.
